Helpers - PIN And Facility Numbers (Boxes 24k, 32, & 33)   Return to Archive

MedLook has always provided options to output a completed HCFA/CMS 1500 form.  A few of the more problematic fields on the form are boxes 24k, 32, and 33, especially in regards to the PIN and facility numbers.  Entering these numbers can be done in two different places in MedLook.  The first place is on the Edit Insurance Carrier dialog.  This is has long been the place to enter and edit these numbers.  Along the very bottom of the dialog there is a place to select the appropriate information and make the edit.  While this method works, it is not very convenient in the sense that you cannot readily see all of your entries and you can forget printing out a list of entries already entered.

Helpers allows for two different grids to enter this same information where you can readily see all of your current entries, print them out, make edits, etc.  The first grid is Insurance/Doctor PINs.  This grid lists the doctor mnemonic and full name, insurance mnemonic and carrier name, insurance number (as a good reference), the provider number, and a check box to designate the entry as a box 24k alternative item.  Entry is simply performed by selecting the appropriate doctor, carrier, and entering the provider id.  If the item is supposed to print in box 24k then that item must be checked as well.  The items can be edited very easily by clicking on the appropriate cell in the grid and making the change required.  The provider number will print in box 33 or box 24k as appropriate.

The second grid is Insurance/Facility #’s.  This grid has the same information as the first except the doctor information is replaced by the facility.  Specifically we have fields for the insurance mnemonic and carrier name, insurance number, facility name, and provider number.  The provider number will print in box 32 as appropriate.

It is generally beneficial to be able to see the list of provider numbers in each grid.  This allows you to easily correct any one item that might be incorrect by only a single letter, etc.  You can also select one or more of the items and use File either Print or Print Preview to print all of the items or only those selected.  It is a good idea to occasionally print this list out and store it in a folder somewhere for safe keeping.
